The faculty of the Department of Human Development and Psychological Counseling believe that many factors must be considered in assessing your application. To aid the admissions committee in examining your goals and reasons for applying, this questionnaire must be completed as part of the admissions process. Each Program's admissions committee meets periodically to make decisions regarding applications and, thus, it is important that you give prompt and careful consideration to these questions.
